Source reported
A record was present in an attributed government, operator or third-party directory. It may be a historical 2024 survey entry, not a current facility assessment.
Independently verified
Current operator, clinical services, licenses, treatment setting, address, certification and/or availability need separate primary evidence. A directory source-check date does not provide that evidence.
Confirm before acting
Contact the provider or responsible licensing authority to verify whether services, clinical supervision, admissions, insurance or housing vacancies are actually offered. Historical detox flags are not medical detox certification.
